FURRY RESCUE FOUNDATION Visitor Waiver Participant Information Full Name* Date of Birth* Date of Visit* Email Address* RELEASE OF LIABILITY, WAIVER OF CLAIMS & ASSUMPTION OF RISK In consideration of being permitted to visit the premises of Furry Rescue Foundation (the "Organization") and to interact with its animals, I, the undersigned participant (or parent/legal guardian if the participant is a minor), agree to the following terms: 1. ACKNOWLEDGMENT OF RISKS I understand and acknowledge that interacting with animals -- including but not limited to dogs, cats, rabbits, and other rescue animals -- involves inherent risks and dangers, including but not limited to: (a) bites, scratches, and physical injury caused by animal behavior; (b) kicks, jumping, or sudden movement resulting in falls or bodily harm; (c) transmission of zoonotic diseases or illness (diseases transmissible between animals and humans, including ringworm, salmonellosis, and parasitic infection); (d) allergic reactions to animal dander, hair, or secretions; and (e) damage to personal property. I voluntarily assume all such risks, known and unknown. 2. RELEASE & WAIVER OF LIABILITY I hereby release, waive, discharge, and covenant not to sue Furry Rescue Foundation, its officers, directors, employees, volunteers, agents, and successors (collectively, the "Releasees") from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury -- including death -- that may be sustained by me or my property while on the Organization's premises or during any animal interaction, to the fullest extent permitted by applicable law. 3. INDEMNIFICATION I agree to indemnify and hold harmless the Releasees from any loss, liability, damage, or cost, including reasonable attorney's fees, incurred due to my presence on the premises or participation in animal interactions, whether caused by the negligence of the Releasees or otherwise. 4. VOLUNTARY PARTICIPATION & COMPLIANCE I certify that my participation is entirely voluntary. I agree to follow all posted rules, staff instructions, and safe animal-handling guidelines at all times. I understand that failure to comply may result in removal from the premises. I acknowledge that I am in good health and capable of safely participating in animal interactions. 5. MINOR PARTICIPANTS If the participant is under 18 years of age, the undersigned parent or legal guardian agrees to these terms on behalf of the minor and assumes full responsibility for the minor's participation and any resulting claims. 6. SEVERABILITY & GOVERNING LAW If any provision of this agreement is found unenforceable, remaining provisions shall remain in full force and effect. This agreement shall be governed by the laws of the state in which the Organization operates.
